We are not able to process your application as it is incomplete. You are missing the following: l A separate and newly completed additional family information form (IMM5406) bearing original signatures for the person(s) you are sponsoring. This must be received at this office by: 06/19/2012.l A completed Sponsorship Evaluation form (IMM5481) for yourself. This must be received at this office by: 06/19/2012.l For all requested forms,questions , must be answered in full. Details must be provided for every month/year since the age of 18. Do not leave any gaps or blanks. Write ‘NONE’ in the box if the question is not applicable to you. All forms must be signed and dated. Incomplete forms will result in a delay in the processing of your application. All requested documents/information and a copy of this letter must be submitted within the timeframes specified above (formatted as “month/day/year”). 2011.11月14日签收，12年1月17日DM1， 2月6日HKAR，4月24日IP，4月27日DM2，5月15日大信封。已经登陆回复: 1.16签收的，收到补资料的平信对啊，你太粗心了.赶紧补料。你现在住哪儿？如果在温哥华我知道有人可以帮忙检查表格的。

夫妻及孩子的CASE，2012.1.16CIC签收，2.26号AR1，4.30平信要求补表，5.26号DM1，6.8北京AR2，6.19IP，6.26补护+补料，7.5护照签收，8.15DM2+双地址，8.18大信封，VO：Figolmn回复: 1.16签收的，收到补资料的平信我也补料了，感觉遥遥无期了
