Page 1: item (11） 父母以前来过加拿大,为什么"Date and place of your last entry to Canad?" 这一栏填不上？ Page2: contact information: 可以填自己的在加拿大的地址？自己算是父母的representative吗？ Page 2 &4: National ID Document expiry date: 父母的身份证是长期的，没有过期日。该如何填这项？ Page 3: dependant UCI: 是填主申请人的UCI吗？ 谢谢！！！

I got this answer from CIC over the phone.National ID expiry date: long term (write it on that space) confirmed with CIC over the phoneDate and place of your last entry to Canada is not required base on the previous check mark you put.
