Document for PERM:1. Resume listing all jobs held (including present position) for past 12 years, including name of employer, job title, dates of employment (mo/yr-mo/yr), and duties; all college/university degrees received, including name/location of school, field of study, and dates attended (mo/yr-mo/yr).2. Photocopies of all diplomas, transcripts, and other educational certificates.3. Educational equivalency evaluation, if available. If you have earned your degree in any country other than the U.S. and have not already obtained an evaluation, we will request one on your behalf.4. Photocopies of any existing employment verification/reference letters or training certificates. 5. Photocopy of each page of your passport that contains any writing.6. Photocopy of each page of your spouse and children’s passports that contain any writing.7. Clear, dark photocopies of all previous U.S. immigration documents, including prior I-797 approval notices, Form(s) I-94, Employment Authorization Documents (work permits), Form(s) IAP-66 or DS-2019 (for J-1 visas), Form(s) I-20 (for F-1 visas), and proof of waiver of foreign residency requirement, Form I-612 (for J-1 visas). Please submit these documents for all family members including yourself.8. Clear, dark photocopies of Form I-94 Arrival/Departure Record currently stapled to your passport (both sides) for yourself and your spouse/children. Please ensure that all expiration dates are legible.Document for AOS (I-485):1. Medical exam and vaccination supplement from an USCIS-Approved Physician (Civil Surgeon). A list of civil surgeons in your area can be obtained at http://www.uscis.gov/civilsurgeons or by calling the USCIS National Customer Service Center at (800) 375-5283. After you have selected English or Spanish as your language of choice for the message, choose #2 for medical examinations from the list of six automated options. You will be asked to key in your zip code to help determine the list of designated civil surgeons close to you. Have pencil and paper ready to record the names of the civil surgeons provided. Please take your vaccination records with you. The results of the medical exam must remain sealed for submission with your adjustment application(s).2. Ten (10) photos taken at an experienced immigration/passport photo shop. Information concerning the photo specifications can be obtained at: http://www.uscis.gov/files/nativedocuments/M-603.pdfNote: you must have Adobe Acrobat Reader installed on your computer to access this link. (Six (6) photos for children under 15).3. A copy of your official birth certificate. NOTE: birth certificates must contain name, date of birth, place of birth, and names of BOTH parents.* If you cannot obtain birth certificates, please contact your assigned paralegal.4. A legible copy of your driver’s license or state ID, if any.*5. A copy of your official marriage certificate, if any.*6. A copy of any divorce decree obtained if you or your spouse were married previously.*7. A copy of your most recent FEDERAL tax return, with all schedules, W-2s, and 1099s. Be sure to SIGN the return before copying. Please do not provide STATE tax returns.8. Legible photocopies of Form I-94 Arrival/Departure Record currently stapled to your passport (both sides) for yourself and your spouse/children. Please ensure that all admission date stamps, status designations, and expiration dates are legible.9. Copies of documents to show that you have maintained lawful status since you began living in the U.S. Please submit copies of all applicable documents in the following list: a. Legible photocopies of your current and expired passports (all pages, including identification pages and all blank pages);b. Form I-20 for all periods in F-1 visa status (both sides);c. Form IAP-66 or DS-2019 for all periods in J-1 visa status;d. Employment Authorization Cards for all periods of employment in Optional Practical Training; ande. The I-797 Approval Notices for EVERY previously approved employment petition.10. If you have been married for less than two years and your spouse is not a U.S. Citizen or U.S. Permanent Resident, evidence that you have a “real and genuine” marriage,* such as:a. birth certificates of any children born of your union;b. jointly filed tax returns;c. recent statements for jointly held bank/credit card accounts;d. phone/utility bills in the name of both you and your spouse;e. lease/deed for real estate/housing in the name of both you and your spouse;f. insurance documents showing either party as the named beneficiary of the other;g. photos of your wedding, family gatherings, travels, etc. including both you and your spouse (photocopies with captions describing events); andh. personal correspondence to one another.
