thanks for lucyfurrer's help a lot, but I still have some question need your help, First I want to know that the document( I-485,)I should sent one I-485 or two I-485 to Tampa,Fl Immigrant office? (because Immigrant office sent me two I-485,two I-131,G-325 two I-693,two I-765 and I-864Form)before to post all the documents whether or not need to contact with tampa Fl Immigrant office?second:I don't know how to fill out I-693 about vaccination question,I thought I have had vaccination history in Beijing,but I really couldn't remember at which year? and what kind of vaccine I have had? it happen that this time I got K1 visa not to have vaccine in Beijing hospital ( was pointed by American consulate)because my body was sensitive problems....now if I chioce in I-693 applicant does not meet immunization requirement )whether or not my application for greencard will be not approval ? thanks again , thanks more people give me more proposal and suggestion.please sent me chinese langues let me understand more well.I appreciate very much !!

I-693表格申请人只填写第一页的第一部分。其余部分由医生填写。查移民局指定医生的网页，输入自己的邮编，会弹出你的居住地周围的指定医生，可以自己选择。https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV在中国打过的疫苗要有疫苗证，没有的话人家不认，还得重新来。比如上了年纪的人手臂上都有打水痘疫苗的疤痕，可以由于那个年代没有预防接种证，所以还得补打。这些表格都只需递交一份。
