我外甥女在中国，因为护照过期了，舅舅打算带她去广州去重新申请护照。但是她的父母必须加拿大填一份ppt028。不过这份表格，有很多看不懂的地方，希望大家指教： 1，宝宝的父亲acknowledge that i am aware that an application for a passport has been made at -----------(name of office). 因为听表姐说宝宝之前做过两次护照，第一次是在多伦多，第二次是在广州，所以不知道上面该填的是哪个地方。 2，I consent to the provision of passport facilities to them upon the application of ---------------(name of person applying for passport) born--------(year, month, day) who is the ---------------(father/mother/guardian) of the said childdeclare thatg to the best of my knowledge and belief, the name of the child is not in any other passport.因为是舅舅要带宝宝去签证，所以不知道上面是不是要填舅舅的名字，还是要填宝宝父母的名字。 所以请大家赐教，谢谢了！
