2. Change “Company X” to your company name under the Model line.
3. Proceed to Appendix A & B for all component submissions
4. Please include all JCS component part numbers that do not contain any substances that appear on the list of chemicals in Appendix A
5. Please include all JCS components part numbers that do contain substances that appear on the list of chemicals in Appendix B
6. If you supply more than one component category (Stamped metal part, Injection molded part, etc) please use one form for each component category
7. Print, sign, include signatory title and date the form to complete
8. Scan signed form and send back to this procurement communication mail box for collection.
