ordered by (name of the customer) whose name is indicated on our invoice number from (date of the invoice).
The products are produced in (name of the country in which the factory is located)
The (name of the product) is intended to be put in contact with:
Dry / Aqueous / Acidic / Alcoholic till % in vol. Oily foodstuff.(choose an applicable purpose),
For (temperature of contact with the food) for a period of (time of contact with the food).
The (name of the product) is /are made of :
(names of the main raw materials); (names of the additives),
And they / it comply/ies to the EU Directives and its amendments and /or the FDA Regulations
Concerning plastic materials and /or final plastic articles intended to come into contact with food.

这个表格，我刚填完，客户那边说这个表格是用于申请认证用，我只填了上下两段，中间那段空着，没有填，没有相关的东西，哈哈哈

traceability code类似于快递的运单号码 可以在相对应的运输公司的网站查到货物的及时信息位置等
