The undersigned (The owner, agent, master or company of the vessel) accordingly declares that the information provided (in response to 1 to 4 above) is correct and that the vessel shall not call or anchor on any ports other than those mentioned above enroute to Jeddah, Saudi Arabia.
This certificate is not required if shipment effected by UNITED ARAB SHIPPING COMPANY (S.A.G) or national shipping company that they identify themselves

把空白处填上 船公司或其代理签章就可以了。拿到正本的船证明去贸促会CCIPT 做文书认证就行了
