The diode should be put across the coil at the relay with the “line” side of the diode on the positive terminal. This allows the energy built up at the coil (when it is energized) to be released at the relay when it is de-energized instead of being transmitted along wires that are connected to the coil. This not only protects the device driving the relay coil from getting this energy (not a problem here since it is just a key switch), but it also reduces EMI generated on these wires, which seems to be the issue here.
A little more investigation yesterday seems to confirm that EMI is the problem. All the tie wraps holding the wires/USB cables under the hub were undone and we ran a separate 79 and 22 wire to the key switch. Without the diode on CR13, if we held the 79 and 22 wires close to any USB device cables at the pod (keyboard, trackball, and/or joystick) while turning the maintenance key on/off repeatedly, it would cause that device or other USB devices to eventually lockup (sometimes just for seconds and sometimes until a reboot).
