手签商业发票5份，显示信用证号，布鲁赛尔税则分类号（？），海关编号，进口许可证号，并声明根据订货形式发票（号？这里表述貌似有问题）（发货），发票必须单独列示离岸价，运费和保险费。

全套不少于2份的清洁已装船海运提单，标记“运费待付”，空白抬头，背书成凭commercial bank of ceylon ltd 指示，显示申请人名称及地址作为被通知人

原产地为中国的证明。签发发票与产品证的联合单据可授受。

这些条款没什么特别的，发票要手签，产地证可单独做，也可以在发票上打上we certify that the above commodity are of china origin,没规定由谁出具，自己做好了，没必有去贸易会和商检。提单consignee 打成to order,提单背面打上to the order of commercial bank of ceylon ltd,紧挨着下面盖上公司的条形单据单，签上字，就ok了。

手签的商业发票 副本5份 标明信用证号码 btn号码 进口许可证号码 并且证明符合indent proforma（大概是简式发票）号码 发票上分别标明价的运费以及保险费用 （感觉这个地方说不通 不知道是什么意思 ）

全套的不少于2份的清洁已装船的运费已付的B/L 指示性抬头 以 commercial bank of ceylon ltd 为指示的背书 NOTIFY PARTY一栏填写开征申请人的名字和地址

包含中国原产地证的联合发票可以接受

6.insurance will be covered by us and policy is attached 保险由我方办理 保险单随证附寄（感觉哟点不对 呵呵）
