因为同事将商品的报关价值些低了，被海关扣住。到不了客户手里。目前卡在海关2星期了，DHL发了一个表格让我填写，我不清楚怎么填。以下是内容：

We understand that Custom has detained the goods viz Bill of Entry/ CSB-II/AWB No. 1 ____________ for the 2__________Commodity value determination___________purpose.
We have gone through the Custom’s Act, rules and all its provisions and wish to avail the benefit of section 124 of Customs Act, 1962 i.e waiver of issuance of Show Cause Notice.
Further we don’t want any personal hearing and give my consent to adjudicating authority to adjudicate the case on documents available with Customs.
We hear by confirm that we accept the value of INR 3 ………………………….. Enhanced/Suggested by customs and we/I are/am ready to pay the duty on the same.
4 Declare about previous offense in customs (if any) or mention “We confirm that we do not have any previous offence in customs”.
We request you to kindly 5 clear/ return the shipment after levying nominal penalty Charges
